Contact your primary healthcare provider to schedule your COVID-19 (Moderna) vaccine at (712) 546-8111. Vaccination appointments are offered directly through our three clinics in Le Mars, Marcus and Remsen.

COVID Testing Options

If you are having respiratory symptoms, please make an appointment with your healthcare provider.  Based on your symptoms, they will determine what test(s) is (are) appropriate for your situation. 

If you are in respiratory distress, please go to the nearest Emergency Room or call 911.

Any citizen may request a home test kit from Floyd Valley Community Health at (712) 546-3335.  You will complete this test in your home and send it to the State Hygienic Lab.  Based on volume, test results will be available in a few days after the test is received.

Due to volume and limited supply at this time, Floyd Valley Healthcare WILL NOT be testing:

  • Asymptomatic patients
  • Employees needing a test to return to work

CDC guidance for returning to work/school does not require testing.  It states:

Beginning at day 6 after a positive COVID test or symptoms start, if you have been fever-free for 24 hours and you are not experiencing symptoms or your symptoms are greatly improving, you may return to work/school.  This would include improvement on any day between day 6 – 10.  On day 11, if fever-free for 24 hours, you may return to work/school.

Travel Testing

Different destinations and modes of transport may require a specific (PCR or antigen) test. Please ensure the test you are scheduling meets the requirements for your individual trip.

PCR TEST: 

Patient swabs are collected at our facility and then sent to Avera. Results are available in 2 – 4 days. Floyd Valley is currently not offering the rapid PCR test.

ANTIGEN TEST: 

COVID Antigen tests are performed at Floyd Valley Healthcare, and results are available the same day.

COVID-19 Vaccinations / Boosters Available

Floyd Valley Healthcare is now scheduling Moderna primary (monovalent) series and booster (bivalent) vaccinations.  The following are the recommendations and local scheduling information:

Moderna

  • The two-shot monovalent series is available to those 6 months of age and older.  The secondary shot should be at least 4 to 8 weeks from the initial vaccination. For those that are immunocompromised, a third primary shot is available, separated by at least 4 weeks for the other doses.
  • Single booster dose (bivalent) of the Moderna COVID-19 vaccine should be administered at least 2 months after completion of the primary (monovalent) series to individuals 18 years of age and older.
  • Appointments are open to the general public through Floyd Valley Clinics. Please call (712) 546-8111 or your provider directly. Please bring your photo ID, insurance card(s) and vaccination card.

Mixing of Vaccines for Boosters

  • Individuals should try to get the same COVID-19 vaccine as their initial primary dose(s), but if it’s not available, it’s okay to mix vaccines. Follow the primary series/dose window. For example:
  • Pfizer primary series with Moderna bivalent booster or vice versa would be administered 2 months after primary series.
  • Pfizer primary series with Johnson & Johnson booster would be administered 5 months after primary series.
  • Johnson & Johnson primary dose with Moderna or Pfizer booster would be administered 2 months after primary series.
  • Monovalent vaccines are no longer authorized as booster doses. If anyone completes a primary series (any vaccine), their booster dose of the Moderna or Pfizer bivalent would be at least 2 months (8 weeks) after their last primary series.

CDC Information on Vaccines and Boosters

As the guidelines for vaccine administration and mask requirements continue to evolve, Floyd Valley Healthcare is working hard to offer vaccination appointments, testing and treatment for COVID-19 as well as up-to-date information for our patients.

Do I need a vaccine if I have had COVID?

We continue to receive questions about the need for the COVID-19 vaccine if you’ve already had COVID-19. The answer is yes! Even if you’ve already had COVID-19, you still need a vaccine. A 2020 study published in the CDC’s Morbidity and Mortality Weekly Report shows that unvaccinated individuals who were previously infected with COVID-19 are still twice as likely to become infected by the Delta variant. The study concludes that all eligible individuals should be offered the COVID-19 vaccine, even if they’ve had COVID-19 before. Additionally, evidence shows that those who were both naturally infected and received vaccination have “super immunity” and much better protection.
