Home / Patients and Visitors / Advance Directives
Floyd Valley Healthcare respects the right of all patients to make informed, autonomous decisions about their care. An advance directive is a legal document that allows you to decide today about your future medical care and only becomes effective when you cannot speak for yourself. Without an advance directive, your provider will turn to your family, starting with your closest relative for decisions, which may result in someone you wouldn’t select making decisions for your care.
When a patient has outlined his or her wishes through an advance directive – or living will – Floyd Valley Healthcare will comply with the advance directive to the extent allowed by Iowa law and in keeping with our mission and principles.
The information contained here is not a substitute for legal advice. While Floyd Valley Healthcare updates this information and forms to keep them current, changes in the underlying law can affect how the form will operate in the event you lose the ability to make decisions for yourself. If you have any questions about how the form will help ensure your wishes are carried out, or if your wishes do not seem to fit with the form, you may wish to talk to your healthcare provider or an attorney with experience in drafting advance directives.
