Floyd Valley Healthcare offers discharge planning services as you prepare to leave the hospital. Your care team will work with you or your family to determine your needs and discuss any concerns you may have about going home, including physical, medical and equipment and aftercare support needs.

If a patient or loved one isn’t able to safely return home, discharge planning can assist in the search for appropriate care, ensuring the transition is smooth and free from anxiety.
